Access to online services

Patients of St Mary’s Surgery aged 16 and over, are able to register to have online access to a number of services.  Patients can manage appointments, order medication and, if they wish, view summary information from their medical record.  Patients who wish to register for these services should register in person at Reception and bring either a photo driving licence or passport as ID.

St Mary’s surgery have also introducd the facility to view limited aspects of your medical record online.  This is known as “detailed coded record access.”   Patients who wish to register for this service must register in person at Reception and bring two forms of ID.  One must be a photo driving licence or passport.  The second must be a proof of address.  The following are acceptable:
 

  • Utility bill issued in the last 3 months
  • Local Authority council tax bill for the current tax year
  • Bank or building society statement
  • Current driving licence (if not already used for photo ID)
  • Mortgage statement issued for previous full year
  • Inland Revenue self-assessment or tax demand
